- Licensure Forms
- Certified Check or Money Order ($55 made payable to NCDPI-Licensure Section)
NO PERSONAL CHECKS
- Official Transcript(s)
(licensure-only students must also submit an official transcript from where received degree)
- Passing PRAXIS II scores if applicable
- Form S - Student Teaching Intern Performance: Evaluation by a NC LEA
